Who We Are

Community Health Connections, Inc. was founded in 2002 to ensure access to health care services for underserved residents of North Central Massachusetts. From our humble beginnings as a small medical clinic serving about 7,000 patients, we have grown into a comprehensive community health center serving over 30,000 patients, providing medical, dental, mental health, oral surgery, behavioral health and low-cost pharmacy services at four locations. Our commitment to the community and to the school-aged children expands beyond the walls of our community health centers with the operation of school based services in 20 local schools (dental and mental health programs) and the formation of a regional school heath coalition.

What We Do

Our Mission is to provide high quality, comprehensive medical and mental health care, preventive and restorative dental care and pharmacy services to all residents of North Central Massachusetts, responsive to a culturally diverse population, without regard to income, insurance status or past medical history; and to participate in health care training programs so as to prepare individuals for careers in family and community health care. Our vision is to eliminate barriers to health care and build a healthier tomorrow for the communities we serve.
